1. Acknowledgment of Risk

I understand and acknowledge that participation in soccer tryouts and related activities involves inherent risks, including but not limited to:

• Physical injury from contact with other participants, equipment, or playing surfaces
• Sprains, strains, fractures, or other musculoskeletal injuries
• Head injuries, including concussions
• Heat-related illness or weather-related conditions
• Other risks associated with athletic activity

I acknowledge that these risks cannot be entirely eliminated regardless of the care taken to avoid injuries.

2. Assumption of Risk

I, on behalf of myself and my child (the "Participant"), voluntarily assume all risks associated with participation in Southern States Soccer Club ("SSSC") tryouts, whether known or unknown, and whether caused by the ordinary negligence of SSSC or otherwise.

3. Release and Waiver of Liability

In consideration for being permitted to participate in SSSC tryouts, I hereby release, waive, and discharge SSSC, its officers, directors, employees, coaches, volunteers, agents, and affiliates (collectively, the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant while participating in tryouts or while on the premises where tryouts are conducted.

4. Medical Treatment Authorization

I authorize SSSC staff to secure emergency medical treatment for the Participant if necessary. I understand that I will be responsible for all costs associated with such medical treatment. I confirm that:

• The Participant is physically fit and has no medical conditions that would prevent safe participation
• I have disclosed all relevant medical information on the registration form
• I will notify SSSC immediately of any changes to the Participant's medical condition

5. Photo and Video Release

I grant SSSC permission to use photographs, videos, or other media featuring the Participant for promotional, educational, or marketing purposes. I understand that no compensation will be provided for such use.

6. Code of Conduct

I agree that both the Participant and I (as parent/guardian) will:

• Treat all participants, coaches, staff, and officials with respect
• Follow all instructions from SSSC coaches and staff
• Comply with all facility rules and regulations
• Refrain from inappropriate language, behavior, or conduct

I understand that SSSC reserves the right to remove any participant or parent/guardian who violates these standards.

7. Insurance

I understand that SSSC does not provide medical or accident insurance for tryout participants. I confirm that the Participant has adequate medical insurance coverage or that I accept full financial responsibility for any injuries sustained.

8. Personal Property

I understand that SSSC is not responsible for any loss, theft, or damage to personal property brought to tryouts.

9. Indemnification

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my or the Participant's participation in tryouts, whether caused by negligence of the Released Parties or otherwise.

10. Severability

If any provision of this waiver is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that this waiver shall otherwise remain in full force and effect.

11. Governing Law

This waiver shall be governed by and construed in accordance with the laws of the State of Mississippi. Any disputes arising from this waiver shall be resolved in the courts of Forrest County, Mississippi.

12. Acknowledgment of Understanding

I HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND SOUTHERN STATES SOCCER CLUB, AND I SIGN IT OF MY OWN FREE WILL.
